Disability facilities
This practice is designed for wheelchair access. We have a wheelchair available if required. Should you have special needs please discuss them with your doctor. There is a disabled car park next to the ramp.
Taxis
Please do not hesitate to ask our reception team to order a taxi from the surgery should you require one.
Prescriptions and Referrals
All scripts and referrals require an appointment with your doctor (in-person, telephone or video telehealth).
Late cancellations / Missed Appointments
At Creek Road General Practice we understand that unforeseen circumstances can occasionally arise that may result in you being unable to attend your appointment at short notice. Unfortunately, when appointments are missed or cancelled at short notice (within 2 hours of the appointment time), there is not sufficient time for the practice to offer the appointment to other patients who have been waiting to see a doctor. When this happens repeatedly, it has a significant negative impact both on your doctor and their other patients.
To ensure that all patients of our practice are able to access timely and high-quality medical care, we require at least 2 hours notice if you are unable to attend any future appointments. With this in mind, missed or cancelled appointments (at short notice) may incur a fee. This fee will be charged at the cost of your missed appointment (a Medicare rebate is not available for this charge).
Privacy
We value the doctor-patient relationship. This practice is bound by the Commonwealth Privacy Act – Privacy Amendment (Private Sector) Act 2000 A copy of our privacy policy is available here.
